Truck drivers’ voices must be heard, and that begins with drivers coming forward and providing their testimony and documented accounts of what is happening within this industry.
SUBMIT YOUR Body Of Evidence 411 Carriers
Only Submit 411 Carrier Information Here
We are building a comprehensive database of Carrier411/FreightGuard reports to better understand how this reporting system affects motor carriers and the trucking industry as a whole. By collecting reports from carriers across the country and analyzing the allegations, reporting patterns, companies involved, and resulting consequences, we can replace individual stories with meaningful, documented data. Our goal is to identify systemic issues, provide carriers with a clearer picture of how these reports are being used, and develop credible information that can support greater transparency, accountability, and fairness for the trucking industry.
We need your help. If your company has received a Carrier411/FreightGuard report, please send us a copy so it can be documented and added to this database. Whether you believe the report was justified, false, misleading, retaliatory, or somewhere in between, we want the data. Every report helps us build a more complete and accurate picture of the system and gives the trucking industry better information to address these issues collectively.

How to Submit Your FreightGuard Report
If a broker has filed a Carrier411/FreightGuard report against your company, you should have received an email notification containing the report and its allegations.
Please follow these steps:
- Locate your FreightGuard notification email. Find the original email sent to your company notifying you that a FreightGuard report was filed.
- Review the complete report. FreightGuard reports may contain multiple responses or comments from both the reporting broker and the motor carrier. Please include all available responses and updates, not just the original allegation.
- Save the report as a PDF. Open the email or complete report, select Print, and choose Save as PDF or Microsoft Print to PDF.
- Include all related correspondence. If additional responses, updates, or notifications were sent in separate emails, please save those as PDFs as well.
- Upload your documents. Submit the PDF file(s) using the upload form below.
Important: Please submit the complete FreightGuard report, including the original notification, reporting broker’s name, report date, allegations, and all subsequent responses or comments from both the broker and carrier, if available. If multiple responses have been exchanged, please include the entire history so we can accurately document the allegations, responses, and any developments that followed.
Why We Need Complete Reports
We are collecting FreightGuard reports regardless of whether the allegations are justified, false, misleading, disputed, or potentially retaliatory.
Having the complete report, including responses from both parties, allows us to better identify reporting patterns, analyze how allegations are presented and disputed, and document the consequences for motor carriers.
Every report matters. Your submission helps turn individual experiences into documented industry-wide evidence.
We encourage carriers to submit their reports even if the matter has been resolved or the report is several years old.
